New Patient Information
As a new patient to Discover Chiropractic, a typical experience would usually involve making an appointment, and then to provide a simple medical history to give the doctors some background information. This can be done ahead of time by downloading and completing our new patient form, or in our waiting room on your intial arrival. Once a treatment room becomes available, one of our friendly Chiropractic Assistants will guide you it, where you can wait for the doctor to tend to you as they review your medical history. When the doctor comes to consult you, they will likely follow up on any notable items on your medical history, and then begin a chiropractic examination.
